Powered industrial truck operators did not receive initial training in accordance with paragraph (l) of this section:  At this jobsite, located at 826 Pennsylvania, Lawrence, Kansas,. The employer had not assured that the forklift operator was competent to operate the SS636 Terex Handlers forklift, which was being used to transport and place material on the upper tier of the scaffold, exposing all employees working in this area to a potential struck by hazard.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports.  At the location of 826 Pennsylvania, Lawrence, Kansas, employees engaged in masonry activities with the use of scaffolding were exposed to fall hazards of approximately 18 feet from the working platform to the ground below. The employees were exposed in that the scaffolds were not completely planked.
