The employee fell approximately 12 feet to the ground floor.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  Site: 815 Broadway Brooklyn, NY  On or about 3/9/17  a) Employees ere not trained to understand and recognize the hazards associated with the use of ladders.
