29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation:  a) On or about February 22, 2017 on the 2017 OSHA 300 log, the employee injury recorded on this date did not include the required detail outlined in section F. The entry did not include the object or substance that injured or caused the illness.

29 CFR 1910.1020(g)(1): The employer did not inform current employees upon their first entering in to employment and at least annually thereafter, of the existence, location, and availability of any records covered by 29 CFR 1910:   a) On or about March 28, 2017, employees of the AASF had not been informed of their right to access exposure records. These records included records for air sampling, water sampling, wipe sampling and noise sampling.
