(a) American Baler Co. on or about March 21, 2017 at the address of 800 E. Center St. Bellevue, OH: The employer did not ensure employees operating forklifts were formally trained in the operation of power industrial vehicles. An evaluation of competency for the employee operating the forklift was not conducted and documented by the employer. The observed operating employee and others working around the forklift were potentially exposed to struck-by injuries due to operation by an unevaluated operator.

Center St. Bellevue, OH: The employer did not ensure forklifts were examined/inspected daily to detect any condition in which would adversely affect the safety operation of the vehicle before being placed in service. Employees operating the units in this condition were exposed to potential injuries associated with equipment malfunctions.
