Worksite located at 2604 N. Baltimore St., Kirksville, Missouri- Employees were exposed to shock or electrocution hazards in that the employer failed to inspect multiple cords that were frayed, grounding pins missing, and outer insulation missing exposing inner wires.

Worksite located at 2604 N. Baltimore St., Kirksville, Missouri- Employees were exposed to shock or electrocution hazards in that employees were allowed to use equipment such as, but not limited to, relocatable power strips and extension cords not designed for construction.

Baltimore St., Kirksville, Missouri- Employees were exposed to a shock or electrocution hazards in that the extension cords employees were plugged into were missing grounding pins, thus the path to ground was not permanent and continuous.
