Worksite located at 1307 NW Broadway, Grain Valley, Missouri- Employees were observed working under elevated employees and  equipment and were not protected from impact by the use of any means of head protection.

Worksite located at 1307 NW Broadway, Grain Valley, Missouri- Employees were exposed to a fall hazard in that the forklift was not designed to support the platform the employees were utilizing.
