29 CFR 1910.147(a)(2)(ii)(B): 29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation.    (a) Fusion Waterjet/Saw Cutting Machine - Serial No. 1777580: The machine operator did not lockout the machine while standing on the cutting table to perform maintenance operations that required him to place parts of his body into the path of operation of the CNC cutting head and into the cutting head enclosure that was open.  This exposed the operator to multiple hazards such as struck-by, caught-in and cutting hazards from mechanical movement.

29 CFR 1910.333(a)(2): Where exposed electrically charged parts were not de-energized safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards:     (a) Fusion Water-Jet/Saw Cutting Machine - Serial No. 1777580: On or about March 22, 2017, an employee/machine operator did not employ electrical safety-related work practices while working on or around the exposed 480 Volt AC energized electrical components within the water-jet/saw electrical enclosure.  This exposed the employee to electrical arc flash hazards and electrical shock hazards.

29 CFR 1910.184(f)(5)(iii): Wire rope sling that had kinking, crushing, bird caging or other damage resulting in distortion of the wire rope structure was not immediately removed from service:  (a) Wire rope sling that had Kinking, crushed areas, multiple broken strands and other damage, was being used in the cutting room to lift slabs  of Granite and other rock countertop material onto the cutting tables of CNC saws and water-jet machines. The sling was not removed from service once found to be defective.

(a) Synthetic web slings located in the stone cutting room were being used for lifting and dragging Granite and other types of countertop material slabs off of delivery trucks and onto the cutting tables. These slings were not tagged or marked with the rated load capacity and type of synthetic web material.

(a) Synthetic web slings that had snags, tears, and cuts and other damage, were being used to pull slabs of Granite and other rock countertop material off of trucks as well as to lift these slabs onto cutting tables of the saws in the cutting room. The slings were not immediately removed from service.
