29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.    (a) Location - 427 Lincoln Street, Waltham, MA.  On or about March 23, 2017, an aluminum ladder jack scaffold platform extended beyond its support greater than 18" inches and was not secured.

(a) Location - 427 Lincoln Street, Waltham, MA.  On or about March 23, 2017, a ladder jack scaffold was erected where the scaffold came within the approach distance of an energized overhead power line.
