29 CFR 1910.303(g)(2)(i)(B): Except as elsewhere required or permitted by this standard, live parts of electrical equipment operating at 50 volts or more was not guarded by suitable permanent, substantial partitions or screens so arranged so that only qualified persons would have access to the space within reach of the live parts. Any openings in such partitions or screens were not so sized and located that persons were not likely to come into accidental contact with the live parts or to bring conducting objects into contact with them:  Worksite located at 417 Thompson, Excelsior Springs, Missouri, employees were exposed to shock and/or electrocution hazards in that live electrical wires were exposed from the ceiling in a hallway passing the publisher's office.  The exposed electrical wires were not guarded by distance as the ceiling is approximately 7 feet from the floor.  This hallway is used for employees to get to the publisher's office, additional work spaces, the break room and rest room.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) On 3/17/2017 employees were exposed to a chemical  known as "Mold Eradicator" in the workplace and were not provided any information or training regarding the hazards associated with the chemical before its use.The employer located at 417 Thompson, Excelsior Springs, MO 64204 failed to develop and implement a written hazard communication which was most recently demonstrated on or about 3/17/2017. Employees experienced irritation/burning sensations to the eyes and mucus membranes; as well as, difficulty breathing.
