29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) On or about 3/21/2017 - 4400 Harpers Road, McCalla, AL, employee(s) filling small holes in the concrete floor were not protected from falls of approximately 10 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a) On or about 3/21/2017 - 4400 Harpers Road, McCalla, AL, the extension ladder used by employees to access the roof did not extend 3 feet or more above the landing.
