29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about 12/8/2016 at 3051 Kumho Parkway Macon, GA, at lines including but not limited to APU II #5, the employer failed to ensure that the interlocked doors were properly utilized where employees were observed inside of the danger areas of the line with interlocked doors defeated that included but was not limited to:  a. The interlocked gate leading to the tunnel was defeated where the key was removed from the gate and inserted into the locking mechanism to complete the circuit allowing the line to continue to run with the gate open.  b. The interlocked gate leading to the catwalk was defeated where the key was removed from the gate and inserted into the locking mechanism to complete the circuit allowing the line to continue to run with the gate open.  c. The interlocked gate leading to the cap-ply festune was defeated where the key was removed from the gate and inserted into the locking mechanism to complete the circuit allowing the line to continue to run with the gate open.   d. The interlocked gate leading to the computer area next to the robot was defeated where the key was removed from the gate and inserted into the locking mechanism to complete the circuit allowing the line to continue to run with the gate open.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about 2/8/2017 at 3051 Kumho Parkway Macon, GA, the employer failed to implement a lockout/tagout program where workers performing maintenance and troubleshooting tasks were exposed to struck-by and caught-in hazards while working on machinery including but not limited to APU II #1, #2, #3, #4, #5, & #6. Additionally, when lockout is required Jang Ho employees rely on Kumho maintenance employees to apply their locks.
