29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Roof - On or about March 28, 2017, employees were not wearing eye protection while using pneumatic nail guns to install shingles on the roof of a residential home, thus exposing employees to possible eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Roof - On or about March 28, 2017, employees installing roof shingles on a one-story residence were exposed to fall hazards up to approximately 17.5 feet onto earthen ground because the workers were not fall protected.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a) Roof Eave - On or about March 28, 2017, employees were using a portable aluminum extension ladder with a warped side rail and was missing the 16th rung at the top of the ladder, thus exposing employees to fall hazards up to approximately 14.5 feet onto dirt ground.
