29 CFR 1910.67(c)(2)(v): 29 CFR 1910.67(c)(2)(v):  A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift.    On or about March 29, 2017, employees were exposed to a fall of approximately 22 feet.  Employees were performing tree removal activities at a residential home located in Peninsula, Ohio 44264.  There was an employee trimming branches from the bucket of a vehicle mounted aerial lift; the employee was not using any form of fall protection.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    On or about March 29, 2017, employees were exposed to being struck by hazards. Employees were performing tree removal activities at a residential home located in Peninsula, Ohio 44264.  There was an employee trimming branches from the bucket of a vehicle mounted aerial lift. There were other employees on the ground working under the bucket without wearing protective helmets.
