29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    (a) At least one (1) employee was working at a job site located at 1230 Ophelia Drive, Forney, TX; was exposed to a fall hazard when working from a single pole scaffold approximately 12-feet to the ground with no fall protection.

29 CFR 1926.1052(b)(3):  Treads for temporary service were not made of wood or other solid material, and were not installed the full width and depth of the stair:    (a) Employees working at the jobsite located at 1230 Ophelia Drive, Forney, TX, accessed the second story using a wooden stairway that had the treads missing. Employees were exposed to a slip and fall hazard.
