29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about March 31, 2017, employees were exposed to head injuries while working below employees performing tear off activities on a residential roof and the employer did not ensure employees wore head protection. 29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  On or about March 31, 2017, employees performing roof tear off operations were dropping material approximately twenty-five (25) feet or greater to the ground with no chute in place, thereby exposing employee(s) to struck-by hazards. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about March 31, 2017, employees were exposed to falls greater than 25 feet while performing tear off activities on a residential roof and the employer did not ensure employees were protected from a fall by the use of guardrail systems, safety net system, or personal fall arrest system.   Preferred Roofing and Siding, LLC was previously cited serious for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 331222 citation number 1 item 2, and was  affirmed as a final order on June 5, 2012, with respect to a workplace located at 2705 Highcrest Ave., Rockford, IL  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
