29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not used:   A) West dissolving tank, on or about March 28, 2017, an employee was performing a hot tap on a Green Liquor piping system and the employer did not ensure the employee(s) were protected from chemical and thermal burn hazards by using appropriate personal protective equipment.                                                                     OR IN THE ALTERNATIE  29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   A) West dissolving tank, on or about March 28, 2017, an employee was performing a hot tap on a Green Liquor piping system and the employer did not ensure the employee(s) were protected from chemical and thermal burn hazards by using appropriate personal protective equipment.
