29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 3, 2017, employees were exposed to struck by hazards. The employees were installing roofing materials on an existing commercial building located in Norton, Ohio 44203.  There was an employee working on the ground working underneath two employees on the roof.  The ground employee was not wearing a protective helmet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about April 3, 2017, employees were exposed to a fall of approximately 10 feet.  The employees were installing roofing materials on an existing commercial building located in Norton, Ohio 44203.  The employees were working on all areas of the roof without using any form of fall protection.
