29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:   (a)  On or about 03/16/17, at the Hyatt Hotel, located at 125 East Main Street, in Rochester, New York, employees were exposed fall hazards from stepping into or tripping in uncovered holes in the floor of their work area.  NO ABATEMENT CERTIFICATION REQUIRED
