29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    a) Boiler Room - On or prior to March 30, 2017, employees were exposed to electrical hazards due to the use of the welding power cord and other power cords not used in accordance with instructions including, but not limited to, not providing Ground-Fault Protection for employees using equipment plugged into the power cords and the flexible power cords were not protected from accidental damage or installed properly.  The welding power cord and other power cords were directly connected to 30 Amp breakers in the 208Y/120V 3PH 4W electrical panel with its cover removed.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:    a) Boiler Room - On or prior to March 30, 2017, employees were exposed to electrical hazards due to the welding power cord outlet and the other power cord outlets directly connected to 30 Amp breakers in the 208Y/120V 3PH 4W electrical panel with its cover removed did not have approved ground fault circuit interrupters (GFCIs) for personal protection.
