29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading:    1) Warehouse Area: On or about March 31, 2017, the employer exposed employees to crush/struck-by hazards by not ensuring the wheels on the tractor trailer were chocked while an employee was unloading pallets of produce using a Toyota Forklift (SN:65952).

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.    a.  Facility - On or about April 5, 2017, employees were exposed to struck-by hazards, in that a Toyota Forklift (SN:68657) was found to be in need of repairs and not in a safe operating condition.    b.  Facility - On or about April 5, 2017, employees were exposed to struck-by hazards, in that a Toyota Forklift (SN:65952) was found to be in need of repairs and not in a safe operating condition.
