29 CFR 1910.1048(d)(1)(i): The employer, having a workplace covered by this standard, did not monitor employees to determine their exposure to formaldehyde.  On or about April 06, 2017 at 5045 Singleton RD, Suite C, in Norcross, Georgia four employees were exposed to chemical hazards associated with formaldehyde use where products including "True Cosmetics" Keratin+ and "RG Cosmetics" Amazon X-treme were used by employees for whom the employer had not performed personal air monitoring to establish exposure levels to formaldehyde.  BIBY'S SALON 3, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1048(d)(1)(i), which was contained in OSHA inspection number 1161624, citation number 1, item number 1 and was affirmed as a final order on 10/04/2016, with respect to a workplace located at 5045 Singleton RD, Suite C, in Norcross, Georgia.

29 CFR 1910.1048(n)(3)(i): The training program for all employees exposed to formaldehyde did not include a discussion of the contents of this formaldehyde regulation and/or the contents of the Material Safety Data Sheet:  On or about April 06, 2017 at 5045 Singleton RD, Suite C, in Norcross, Georgia four employees were exposed to chemical hazards associated with formaldehyde use where products including "True Cosmetics" Keratin and "RG Cosmetics" Amazon X-treme were used by employees for whom the employer had not informed the affected employees of the requirements of this standard or the associated safety data sheets.  Biby's Salon 3, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1048(n)(1), which was contained in OSHA inspection number 1161624, citation number 1, item number 2a and was affirmed as a final order on October 04, 2016 with respect to a workplace located at 5045 Singleton Road, Suite C, in Norcross, Georgia.

On or about April 06, 2016 at 5045 Singleton RD, Suite C, in Norcross, Georgia four employees were exposed to chemical hazards while using "True Cosmetics" Keratin+ and "RG Cosmetics" Amazon X-treme containing formaldehyde and the employer did not provide affected employees with effecting hazard communication information and training.
