29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4-feet (1.2 m) or more above a lower level is protected from falling by a guardrail system, safety net system, or personal fall protection system.    On or about April 6, 2017, employees were exposed to a fall hazard approximately 9 feet while in the loft storage area and the employees were not protected from falling by a guardrail system, safety net system, or personal fall protection system.

29 CFR 1910.178(l)(6): The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  On or about April 6, 2017, the employer did not document the required certification for each operator which includes; the operators name, date of training, date of evaluation and/or the name of the person conducting the evaluation.
