29 CFR 1926.502(b)(1): The top edge height of top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:      Instances including, but not limited to, the following:    a) Job site, 506 West North Avenue, Pittsburgh, PA: On or about April 5, 2017, during construction activities near the penthouse entry area, the employer did not ensure that appropriate fall protection systems/devices were in use in that an existing section of a parapet wall was relied on for fall protection, but it was determined to be only approximately  26 inches tall, exposing an employee to a fall hazard of approximately 27 feet.    b) Job site, 506 West North Avenue, Pittsburgh, PA: On or about April 5, 2017, during construction activities near the wooden rooftop walkway between buildings 508 and 506, the employer did not ensure that appropriate fall protection systems/devices were in use in that an existing section of a parapet wall was relied on for fall protection, but it was determined to be only approximately 25 inches tall, exposing an employee to a fall hazard of approximately 12 1/2 feet.
