29 CFR 1926.502(b)(1): The top edge height of top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:      Instances including, but not limited to, the following:    a) Job site, 506 West North Avenue, Pittsburgh, PA: On or about April 3, 2017, during construction activities along the eastern part of the roof across from the penthouse, the employer did not ensure that appropriate fall protection systems/devices were in use in that existing parapet walls were relied on for fall protection, but it was determined that their height was approximately 30 inches tall, exposing employees to fall hazards of approximately 27 feet.    b) Job site, 506 West North Avenue, Pittsburgh, PA: On or about April 3, 2017, during construction activities along the parapet wall above a void between buildings 506 and 508, the employer did not ensure that appropriate fall protection systems/devices were in use in that existing parapet walls were relied on for fall protection, but were approximately 25 inches tall, exposing employees to fall hazards of approximately 12 1/2 feet.    b) Job site, 506 West North Avenue, Pittsburgh, PA: On or about April 3, 2017, during construction activities along the parapet wall by the penthouse entry door, the employer did not ensure that appropriate fall protection systems/devices were in use in that existing parapet walls were relied on for fall protection, but were approximately 26inches tall, exposing employees to fall hazards of approximately 27 feet.
