29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not de-energized and grounded, nor were other protective measures provided before work was started:  a. On or about April 5, 2017, at the north side of the residence, employees were performing tree trimming work near energized overhead lines and the employer did not de-energized and grounded the lines before the work started exposing the employees to electrical hazards.

29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained:  a. On or about April 5, 2017, at the north side of the residence, employees performing tree trimming work were exposed to electrical hazards when the boom became in contact with energized overhead power lines.

29 CFR 1910.333(c)(3)(iii)(B): Employees standing on the ground would not contact the vehicle or mechanical equipment or any of its attachments:  a. On or about April 5, 2017, at the north side of the residence, employee on ground contacting the boom truck was exposed to electrical hazards when the boom became in contact with the energized overhead power lines. The employer did not ensure employees working on the ground did not contact the vehicle while being operated near energized power lines.
