29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a) At the construction of the Preserves at the Villages in Bradenton, where employees were using personal fall arrest systems while pouring cement on the balcony of the second floor. No written certification of the fall protection equipment training was available As observed on or about 04/05/2017.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) At the construction of the Preserves at the Villages in Bradenton, where the side rails of a portable ladder used to gain access to the second floor didnt extend at least 3 feet (.9 m) above the upper landing surface. As observed on or about 04/05/2017.
