The eye wash equipment required a separate motion by the operator to remove a dust cover in the event of an emergency and presented contaminates in the flushing fluid.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):    Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 11, 2017, the employer did not ensure that the Komatsu forklift (model FB15MH-2; s/n 009650A) was removed from service during a time in which it was not in safe operating condition.  Employees were exposed to struck-by injuries in that the horn was not operative and a leak in the side-shifter hydraulic cylinder presented the risk of unstable loads.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 11, 2017, employees that operated the stripper at Cell 1, Line 2, were exposed to pinch point hazards created between the machine's ram and fixed parts as the ram lowered.  The machine was not adequately guarded to protect employees from the pinch point.    b. Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 11, 2017, employees that operated the stripper at Cell 2, Line 1, were exposed to pinch point hazards created between the machine's ram and fixed parts as the ram lowered.  The machine was not adequately guarded to protect employees from the pinch point.    c. Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 11, 2017, employees working at the Cell 2, Line 1 stripper machine were exposed to pinch point hazards created between pallets and the conveyors frame as a pallet advances forward toward the operator.      d. Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 21, 2017 the employer did not ensure that the Wellsaw (model V-20; s/n 2234) band saw #OP 140 was adequately guarded.  Employees that operated the saw were exposed to amputation injury in that the unused portion of the blade was not guarded.    e. Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 11, 2017, the employer did not ensure that the band saw used at the Cell 3 knockout area was adequately guarded.  Employees that operated the saw were exposed to amputation injury in that the unused portion of the blade was not guarded.      f. Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 21, 2017, employees operating the OD Grinder machine located at the Cell 2 knockout area were exposed to rotating parts hazards.  The machine was not guarded in that employees were not prevented from accessing the hazard area as the machine completed its cycle.
