Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 21, 2017 the employer did not ensure that the Wellsaw (model V-20; s/n 2234) band saw #OP 140 was adequately guarded.  Employees that operated the saw were exposed to amputation injury in that the unused portion of the blade was not guarded.    e. Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 11, 2017, the employer did not ensure that the band saw used at the Cell 3 knockout area was adequately guarded.  Employees that operated the saw were exposed to amputation injury in that the unused portion of the blade was not guarded.      f. Ross Casting & Innovation, LLC - Sidney, Ohio: On or about April 21, 2017, employees operating the OD Grinder machine located at the Cell 2 knockout area were exposed to rotating parts hazards.  The machine was not guarded in that employees were not prevented from accessing the hazard area as the machine completed its cycle.
