29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:    a. On or about April 12, 2017, at the second buck of the scaffold, the employer did not ensure that the platform units were installed so that the space between adjacent units were not more than 1 inch wide, exposing employees stuccoing a wall to an 14 feet, 4 inches fall hazard.

On or about April 12, 2017, at the second buck of the scaffold, employees applying stucco to a wall of the building were working from a wood board that was 9.5 inches wide, being exposed to an 16 feet fall hazard.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling:  a. On or about April 12, 2017, at the scaffold, the front edge of all platforms were 18.5 inches or more from the face of the work and no guardrail system had been erected along the front edge of the platform or fall protection was in used, exposing the employees applying stucco to a wall to a 16 feet fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access.  a. On or about April 12, 2017, at the scaffold, employees stuccoing a wall were exposed to a fall hazard when ladder was not installed to climb up and down from the scaffold platforms. The employees were using the scaffold frame to climb the scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a.  On or about April 12, 2017, at the second buck of the scaffold, employees applying stucco to a wall were not protected from falling 16 feet to the ground below by the use of a fall protection system while working from the wood boards that were place between the window openings and the mid-rail of the scaffold.
