29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Central Machinery Drill Press: On or about April 11, 2017 and times prior, employees used the Central Machinery Drill Press, Serial Number 355851502, to drill holes in component boxes to install wiring. The rotating drill bit was not provided with a shield guard to prevent employee exposure to the flying chips and rotating Parts.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) Drill Press Room: On or about March 17, 2016 and times prior, an employee used a Central Machinery 8 inch abrasive bench grinder to smooth off sharp edges on the din rails. The adjustable work rest was not adjusted on the right side resulting in a 1/2 inch opening between the wheel and the work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Drill Press Room: On or about March 17, 2017 and times prior, an employee used a Central Machinery 8 inch abrasive bench grinder to knock the spurs off cut din rails. The grinder did not have the adjustable tongue guard installed correctly resulting in a 1 1/2 inch opening between the abrasive wheel and the top of the grinder.
