29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:  JOBSITE:  The employees, engaged in servicing of an A/C unit within very close proximity of the open-sided flat roof were not protected from the hazard of falling approximately 25 feet to the ground by the use of guardrails, personal fall arrest systems, or other equivalent means.
