29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  a.  On or about April 12, 2017, at the above address, an employee was required to operate a powered industrial vehicle and had not been provided with training, including but not limited to formal instruction, practical training and an evaluation of the operators performance, thereby exposing employees to struck by and crushing hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  a.  On or about, April 12, 2017, at the above addressed jobsite, employees conducting masonry operations from a tubular welded framed scaffold, sixteen (16) feet above the ground, were exposed to fall hazards due to the lack of a complete guardrail system on a tubular welded scaffold.  MBBS Masonry Group, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1) which was contained in OSHA inspection number 452294, citation  number 1 item number 3 and was affirmed with a final order date of July 17, 2012, with respect to a workplace located at 14 4th Street, Hinsdale, IL 60521.
