29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems:  A) North side of building, on or about April 11, 2017, an employee had to traverse an area between a stack of blocks and an open sided floor approximately one to two feet wide in the hoist area to retrieve the guardrail cables and then cross back over the same exposed area to re-attach the guardrail cables, exposing the employee to an 11 foot fall hazard.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks:   A) North side of job site, on or about April 11, 2017, employees operated a JLG forklift #10042 that did not have an operable horn, exposing employees to a struck-by hazard.
