29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  a)  130 Park Avenue, Florham Park, NJ.  Employee working from the basket of a telescoping boom lift did not have his lanyard tied off to the aerial lift's anchorage point exposing him to falls from being catapulted out of the basket.                                                   Violation observed on or about 04/12/2017.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)  130 Park Avenue, Florham Park, NJ.  Employee placing blocking between joists was exposed to falls of 46 feet to the ground below from the top level  and 8 feet to the floor below as no methods of fall protection were being used.                                                     Violation observed on or about 04/12/2017.

29 CFR 1926.501(b)(14):   Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:  a)  130 Park Avenue, Florham Park, NJ.  Employees sealing windows were exposed to falls of 10 to 27 feet as they were defeating the guardrail fall protection as they were straddling the wall opening guardrail while doing their work.                                                      Violation observed on or about 04/12/2017.
