29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a.   On or about April 11, 2017, at the jobsite located at 12451 Shady Bridge Trail, Jacksonville, Florida, the employer failed to perform an inspection to identify hazards and unsafe work practices that exposed employees installing lathing to fall and ladder hazards.

On or about April 11, 2017, an employee installing lath above the 4:12 pitch patio roof was exposed to a 12 feet fall hazard in that he was not protected by a fall protection system.
