29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about at 4/11/17 located at Unite # 25. 4165 N. Peachtree Rd., Chamblee, GA: Employees were framing and installing the 2nd floor walls on an apartment duplex that was under construction while working at the edges of the staircase floor hole opening, the employer did not ensure that guardrails or floor hole coverings was used by the employees while exposed to fall hazards of approximately up to 10.4 - feet.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:   On or about at 4/11/17 located at Unite # 25. 4165 N. Peachtree Rd., Chamblee, GA: An employee was taking a measurement to frame and install the 2nd floor stairwell case on an apartment duplex that was under construction while using an 8- foot steep ladder and not having a three point contact resulting in him falling to the floor and 911 transported him to the hospital with a fractured back.
