29 CFR 1910.1030(f)(2)(iv): The employer did not assure that employees who decline to accept hepatitis B vaccination offered by the employer sign the statement in Appendix A:  At 4140 Forest Park Avenue, St. Louis, MO, employees tasked with being occupational first aid responders were offered the opportunity to accept hepatitis B vaccinations.  The employer did not provide a declination statement to those employees who declined the vaccinations.  This violation was noted on 04/18/17.
