29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  4890 Perkiomen Avenue Reading, PA - The employer did not provide training to employee(s) which would have allowed employee(s) to recognize the hazards of falling and the equipment and procedures to use to minimize those hazards, thereby exposing employees to possible injuries, on or about April 12, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  4890 Perkiomen Avenue Reading, PA - Employees were observed using an aluminum extension ladder that did not extend at least 3 feet above the landing surface to the building, thereby exposing employees to a fall hazard of approximately 16 feet to the ground below, on or about April 12, 2017.  No abatement certification or documentation required.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  (a)   4890 Perkiomen Avenue Reading, PA - Employees were observed using an aluminum extension ladder that did not extend at least 3 feet above the landing surface to the building, and the employer did not provide ladder training to the employees to minimize hazards related to ladders thereby exposing employee to an approximate 16 foot fall hazard to the ground below, on or about April 12, 2017.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a)  4890 Perkiomen Avenue Reading, PA -The employer did not ensure that employees working on the roof framing and performing finish work, were protected from falls, with guard rails, safety nets or personal fall protection, thereby exposing employees to a fall hazard of approximately 16 feet, on or about April 12, 2017.      No abatement certification or documentation is required.  PLS Drywall & Ceilings Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(1), which was contained in OSHA inspection number 1088031, citation number 1, item number 2 and was affirmed as a final order on September 21, 2015, with respect to a workplace located at 1500 Walnut Avenue, Dalton, GA 30720.
