29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   The employer is failing to train employees on recognition of fall hazards and the use of fall protection.  The most recent occurrence of this was observed on April 17, 2017 at the home located at 15457 Laurel Circle, Omaha, NE where the employer has a crew of 5 inadequately trained employees exposed to fall hazards while re-roofing an existing residential home.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems.                                                                                                          The employer is failing to protect employees from fall hazards associated with performing residential construction activity while exposed to falls 6 feet or greater.  The most recent occurrence of this was observed on April 17, 2017 at the home located at 15457 Laurel Circle, Omaha, NE where the employer has a crew of 5 employees 4430 N. 143rd Street, Omaha, NE to fall hazards while re-roofing an existing residential home.     Saul Navarro-Alvarez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1170875, citation number 1, item number 1 and was affirmed as a final order on September 13, 2016, with respect to a workplace located at .
