29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite: On or about April 18, 2017, an employee was working from the roof and he was not protected from falling from the roof. The fall hazard to the employee was about 25 feet to the ground. The pitch of the roof was 6 in 12.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill: (a) Jobsite: On or about April 18, 2017, an employee needs to be retrained so that he understands when and how to use fall protection when working from a roof.
