29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    Instance a. Site - On or about April 12, 2017, the employer failed to provide fall protection to employees who were engaged in framing activities and exposed to falls of over 10 feet.    Instance b. Site - On or about April 12, 2017, the employer failed to protect the floor opening around the staircase to prevent employee falls of over 8 feet.

Site - On or about April 12, 2017, the staircase in use by the employees to access the second floor level was not provided with at least one handrail on either side of the staircase.
