29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about April 17, 2017, employees were installing a water line in an excavation approximately 80 feet long and 8 feet deep without having all workers trained in recognizing and avoidance of such hazards as, but not limited to electrical shock hazards and the regulations in excavation safety such as, but not limited to cave-in protection and adequate egress.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about April 17, 2017, the employer was using a portable sump pump to remove the water from the 8 foot deep trench with an extension cord that no longer had strain relief and the cable insulation was pulled away showing the copper wiring, exposing employees to a shock hazard.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about April 17, 2017, employees were working in an excavation approximately 8 feet deep and 80 feet long with only one ladder causing employees to have to travel more than 25 lateral feet to exit the excavation, exposing employees to the hazards of delayed/impeded egress in the event of a cave-in or other emergency.
