OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were not protected from crushing hazards while using a below-the-hook lifting device:     On or about April 11, 2017, employees were exposed to struck-by and caught-between hazards while using the EM-12 Magnetool magnet.  Two pieces of steel were being moved into a grinder with the EM-12 Magnetool magnet when the pieces fell off the magnet, resulting in a partial fingertip amputation.    Among others, one feasible and acceptable method of abatement would be to ensure loads are centered on the magnet to allow for full lifting capacity of the magnet in use, and to follow the manufacturer's instructions to prevent air gaps between the magnet and the load.  Additional means of abatement include following work practices of ANSI B30.20-2013 for Below-the-Hook Lifting Devices, including sections 20-3.4.4 and 20-3.5.

On or about April 17, 2017, the employer did not ensure that machine specific energy control procedures were developed, documented, and utilized to control hazardous energy when employees accessed the hazardous area of, including but not limited to the Blanchard Model K (Serial #12447) grinder to change grinding stones.  b.)    On or about April 17, 2017, the employer did not ensure that machine specific energy control procedures were developed, documented, and utilized to control hazardous energy when employees accessed the hazardous area of, including but not limited to the Blanchard Blue #60 grinder to change grinding stones.          Precision Steel Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(I) which was contained in OSHA inspection number 330584, citation number 1, item number 2a and was affirmed as a final order on September 23, 2013 with respect to a workplace located at 31 E. Sylvania Ave. Toledo, OH.

On or about April 19, 2017, the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to change stones on the Blanchard grinders, and are exposed to amputation and caught by hazards from rotating parts in that the equipment was not properly deenergized.      Precision Steel Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7)(I) which was contained in OSHA inspection number 330584, citation number 1, item number 3 and was affirmed as a final order on September 23, 2013 with respect to a workplace located at 31 E. On or about April 19, 2017, the employer did not ensure employees were protected while operating the Blanchard Grinder HD #3 (Serial #14446). Employees were required to use the grinder to smooth out steel parts, and are exposed to hazards from the moving parts of the table and/or grinding stones.      b.)    On or about April 19, 2017, the employer did not ensure employees were protected while operating the Blanchard Grinder Blue #60. Employees were required to use the grinder to smooth out steel parts, and are exposed to hazards from the moving parts of the table and/or grinding stones.      c.)    On or about April 19, 2017, the employer did not ensure employees were protected while operating the Blanchard Grinder Model K (Serial #12447). Employees were required to use the grinder to smooth out steel parts, and are exposed to hazards from the moving parts of the table and/or grinding stones.      Precision Steel Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA inspection number 330584, citation number 1, item number 6 and was affirmed as a final order on September 23, 2013 with respect to a workplace located at 31 E.
