29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On 4/7/2017, at the Greystone Lot #32 job site, the employer did not perform frequent and regular safety inspections of the job site so as to prevent hazards employees and subcontracted employees may be exposed to including scaffold and fall hazards.

29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut.  On 4/7/2017, at the Greystone Lot #32 job site, employees were exposed to amputation hazards as the Bosch table saw used at the job site was not equipped with a guard/hood.
