North side, Sewing Floor, Second Row, Station 1- Brother sewing machine. ii. North side, Sewing Floor, Second Row, Station 2- Brother sewing machine. iii. North side, Sewing Floor, Second Row, Station 2- Juki sewing machine. iv. Northwest side, Forum Jacket area, Station 4- Juki sewing machine. v. Southeast side, Arjo Station -Brother sewing machine. vi. Southwest side, Sewing floor, First Row, Station 4- Brother sewing machine. vii. Southwest side, Sewing floor, First Row, Station 5- Brother sewing machine. (b) Sewing Floor, All Stations: On or about April 19, 2017, employee(s) were exposed to struck-by hazards from the operation of Brother Sewing Machines and Juki Sewing Machines without eye protection covers to prevent eye injuries from broken needles.
