OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards:  a:  On or about April 19, 2017, at Eddie English Co, Inc., on the outside of the shop on the southwest side, employees were exposed to crushing injuries from the hazard of falling loads in the event that lifting beams (spreader bar) being used would be overloaded.  The employer was unaware of the rated load capacity as well as the weight of the spreader bar, as the employer failed to ensure that this information was marked on the main structure of the spreader bar through the use of a nameplate or other permanent marking.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  a.  On or about April 19, 2017 in the main shop on the east side, employees were using a Rockwell radial arm drill press that was not guarded by a barrier around the point of operation, exposing employees to cuts, bruises, and struck-by hazards.  b.  On or about April 19, 2017 in the main shop on the east side, employees were using a Leland/Gifford Four Head drill press that was not guarded by a barrier around the point of operation, exposing employees to cuts, bruises, and struck-by hazards.

On or about April 19, 2017, in the rear of the shop on the east side, an employee was using a Wysong and Miles Company plate shear machine that was not guarded to keep the operator's body and hands away from the point of operation, exposing employees to cuts, bruises, and amputations.

Inc. on the south side of the shop, the cables on a Miller Millermatic 350P welder used to weld medium to heavy galvanized steel while repairing and fabricating trailers had a damaged insulation cable that had not been replaced, exposing employees to explosion hazards.
