29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a)  On or about April 6, 2017, Topy Precision Mfg., Inc. failed to ensure that all areas where hazards created by ingoing nip points, pinch points, and rotating parts were guarded on the Double Disc Grinder #2 to prevent injury to employees during operation.      Topy Precision Mfg., Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1018385, citation number 1, item number 6 and was affirmed as a final order on July 21, 2015, with respect to a workplace located at 1375 Lunt Ave., Elk Grove Village, Illinois 60007.     In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
