29 CFR 1910.147(c)(2)(ii): An energy isolating device that was capable of being locked out, did not utilize a lockout device, which rendered the machine or equipment inoperative.    Employees of Permian Tank & Manufacturing performing maintenance on equipment are exposed to caught-in and struck-by hazards because devices capable of being locked out did not utilize a lockout device in two separate locations:    a). The breaker box for  the roll machine in the code shop    b). Two breaker boxes in the satellite shop.

The breaker box for  the roll machine in the code shop    b). Two breaker boxes in the satellite shop.
