29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a) East side of building - On or about April 12, 2017, employees were working from a pump jack scaffold with missing components and physical defects, approximately 35 feet above grade, that had not been inspected by a competent person prior to use.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.  a) East side of building - On or about April 12, 2017, employees were working from a pump jack scaffold,  approximately 35 feet above grade, at a location where the front edge of the platform was approximately 23 inches from the face of the work.
