29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a) Building 1607, Fort Eustis, VA 23604: On or about April 18, 2017, and times prior thereto, subcontractor employees were permitted to perform roofing work and handle materials including (but not limited to) 8' eave trim sections, in proximity to 120/240 volt energized power lines and to wear fall arrest equipment as a means of fall protection while working above the same energized power lines, exposing employees to electrical hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could neither free fall more than 6 feet (1.8 m) nor contact any lower level:  a) Building 1607, Fort Eustis, VA 23604: On or about April 18, 2017, and times prior thereto, subcontractor employees that performed roofing work wore personal fall arrest systems that were rigged such that the employees could contact a lower level, including 120/240 volt energized power lines, exposing the employees to fall hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
