29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) on the jobsite - employees were not protected from falling through the skylight opening, in that, personal fall arrest systems were not used, there was not an adequate cover, and there were no guardrail systems in place, exposing employees to an 18.5 ft fall hazard; on or about 4/17/17.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. (a) on the jobsite - employees were exposed to 9ft-9in and 10ft fall hazards over the edge of the roof, in that, fall protection was not used at all times when on the roof; on or about 4/17/17.
