29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  (a) on the jobsite - employees were not protected from falling through the skylight opening, in that, personal fall arrest systems were not used, there was not an adequate cover, and there were no guardrail systems in place, exposing employees to an 18.5 ft fall hazard; on or about 4/17/17.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) on the jobsite - employees were exposed to 9ft-9in and 10ft fall hazards over the edge of the roof, in that, fall protection was not used at all times when on the roof; on or about 4/17/17.
